My instagram was hacked

I can't get into my Facebook account. It was linked to my personal Instagram. Somehow, another Instagram was added to my account (the one in the pic), then disabled, and now because that was disabled, I can't get into facebook. Whenever I try to make an appeal, all of the links don't work and I can't find the form to dispute it. Please help.
